 Sickening scenes? Watch a film called, Ichi the Killer. Its full of them. Got to be the most violent movie i've seen in years!
When Kathy Bates chops his feet in Misery.
The opening 20 minutes of Saving Private Ryan.  In reality it must have been hell on earth for some very courageous men.

The eyeball slice in un chien andalou was pretty sick, though the whole film is pretty weirdly disturbing and might head-sicken you, though if it makes sense to you it wouldn't have the same effect as it had on me. I had no bloody idea what I was going on.

The tooth-drilling scene in "Marathon Man" is pretty cringe-worthy.

Most of the killings in Seven. But the gluttony one wins it for me.

Woody Harrelson getting it on with his landlady in Kingpin.

I'd also agree with the Misery ankle smashing bit.

Although I know this is a very real experience for some people, I was most shocked when I very first saw Scum, including the "Where's yer tool?" bit and the greenhouse bit.  I couldn't sleep for nights after the greenhouse bit because I was so young when I watched it (about 8 - me and my friend pinched it out of her big brothers room) I'd never even considered that things like rape happen to boys.  Sorry, if that going off the point!

Oh, and the eyeball squishing bit in Kill Bill 2.

Mine would have to be Seven.  Although you don't see a whole lot, I find it very distrubing with the scene of the man raping with women with the long strap on knife thing...

Just hearing the noise of the guys face on the curb made me feel sick. I think it helps that we didn't see it as the power of suggestion is mighty stronger than the actual vision as we are constantly being de-sensitised by the news and the ready access to carnage on the internet.

What happened to him?  Did he die or was he maimed/disfigured ?
Although you don't actuall see it happen, I always wince at Mr. White cutting off the coppers ear in Reservoir Dogs.
