it's the first of your options - it's staffed (it's owned by Amazon.com).

go to the site, click 'help' and 'about IMDB' and there's loads of info including a detailed history.

Although primarily edited privately (they have a UK office in Bristol), anyone (including filmakers I guess) can contribute.

Never done it myself (apart from the odd review) but I know someone who has submitted corrections/updates to the "The Last Unicorn (2006)" information.
