Gulf War veteran Adrien Brody is sent to an asylum for a crime he didn't commit.
Pumped full of drugs, trapped in a straight jacket and placed in a mortuary drawer on a nightly basis by nasty Kris Kristofferson, Brody is able to flash-forward in time, where, in 2007, he meets Keira Knightley, whom he had encountered as a young girl in 1992, prior to his incarceration in the asylum.
A disbelieving Knightley initially tells Brody that he cant be who he claims to be, as he died on January 1st 1993, but she doesnt have any details of his death. We later learn it was due to a head wound.
In a return visit to the future, Brody convinces Knightley that he is who he claims to be, and she agrees to help him piece together his past.
Brody learns from the adult, screwed up Keira Knightley of 2007 that her mother (Kelly Lynch) died in tragic circumstances. The death of her mother marked the start of Keira Knightleys eventual decline to her future heavy-drinking, miserable, lonely life.
Learning that Kelly Lynch will pass out one day with a cigarette in her hand, and burn to death from the resulting fire, Brody gives her a letter warning her of this on the afternoon of January 1st, 1993.
Returning to the asylum that evening, Brody slips on ice and hits his head, causing his fatal head wound. Asylum staff place him into the drawer before he dies, allowing him to return to 2007 and be reunited with Knightley.
In this altered future, having been convinced by Brody to believe the contents of the letter, Lynch has (presumably) cleaned up her act, so that in the improved 2007, she is still alive and it is apparent that she has a strong relationship with her (no longer dysfunctional) daughter.
Brody and Knightley meet and drive off together, and I guess live happily ever after
