For more on marking an answer as the "Best Answer", please visit our FAQ.I would suggest that you get yourself the good old Haynes manual. It will save you a fortune and show you how to do the discs. I haven't done a 2001 Transit discs, but most discs are pretty straight forward.
Jack up, wheel off (axle stands of course). Remove the brake caliper (usually two or three bolts on the inner side). Hang the caliper from the shock spring etc with wire (so you don't strain the pipe). The disc is often held on with one or two cross head screws. Undo these (usually rusted so an impact driver is handy). Fit new discs and reverse the above procedure.
You may need to push the pads in (to make way for the extra thickness of the new disc). Be careful and do not damage it if you use a lever of some sort. That's it really. Any doubts, call someone in the know.
