Mercedes Alternator etc Belt

My Sprinter 308D (1997) won't keep its belt in place. We have re fitted it 5 times tonight and turning over by hand all appears ok but start the engine and off it comes. All pulleys appear in line with nothing loose and it seems tensioned ok. Anybody got any ideas as to what might be wrong?

has the bearing gone in the tensioner? is a pully off line has the nut holding the tesioner come loose?

i had a recent problem when the belt wouldnt sit properly, it never came off but it was always running half on..........i found the bold holding the tensioner on was loose??
