alan - you omitted to ask a question.
You based your conclusion on no evidence and no knowledge of the accident, except for an article you read in the press.
You assume that accelerators don't stick. I may have saved your life by demonstrating that yes, they can stick and giving you information how to deal with it.
I conclude my replies are extremely constructive, unlike your biased and uninformed opening post.

as for the first link you posted the guy looks like an idiot and as the police say they've never had this before and were going to look into it further, i don't suppose you have an update on that story

the other possibility is it was an auto driven by someone not used to one and the buried the throttle to the floor thinking it was the brake, harder they braked the faster it went, i know it sounds daft but it does happen.
As regards throttles sticking open it really is a very rare occurance usually caused by something else like the pedal catching under a mat or carpet , A few years back there were problems with Ford Explorer (in the States )throttles staying open i believe there was at least one person killed i think that ended up as an issue with the carpet holding it down.

I've had many problems with Transit vans.
1. Had a sticking accelerator once. To 'release' it, I actually had to depress it a bit further first!
2. A scary moment once when my shoelace caught on the accelerator pedal and I couldn't get my foot on the brake - fortunately managed to swing into a side road and pull hand brake.
3. Shed a cam belt at top of steep hill. It provides power to steering and brakes....!
