He is not mad, but, if he were, he'd still have to get past his generals before his wishes were carried out. They are certainly not stupid enough to attack American bases.

It's all for the benefit of his people; easier to paint a picture of their being under threat from the West than to feed them properly..

It's just 3 years since one of their submarines torpedoed a South Korean naval vessel and 46 sailors were killed. Sometimes they get carried away with all the sabre rattling.
