In that sense the fuss was about breaches of privacy in a case where it was reasonable to assume privacy. Had the D. of C. gone topless on a public beach it might not nearly have caused so much of a stink.
Rather like the difference between those photos of the Duchess and Prince Harry's strip billiards incident, when he (or more accurately his bodyguards) allowed a breach of privacy leading to embarrassing photos. The Duchess's privacy was actively invaded when it should not have been. David Cameron waived his right to privacy by appearing in public.
