It's a sad fact about fame and / or power - the individual becoming more powerful, and looking and behaving in a more ludicrous, or dangerous-to-others manner, has ensured that during their rise, they are increasingly surrounded only by people who agree with how they look and / or what they do.
This applies to people like Michael Jackson, Harvey Weinstein etc., and of course, to the President.
People who surround the President either love and accept him for who he is and how he looks, or work for him, and wish to continue working for him.
That precludes either set from having a quiet, but frank word, from which the President would assuredly benefit.

I notice that the President's recorded response is "Fake news ..." which is his knee-jerk response to anything either that criticises him and his approach to life in general, and his presidency in particular, or to anything that he actually doesn't understand in the slightest.
Since these two categories occupy his entire existence one way or another, or even both together, it's a fail-safe buffer between himself and reality, which is why he trots it out on a daily basis.
