It was a mistake. Similar incidents have happened before. During the 1994 Final coverage, the BBC missed one of Manchester United's goals because they were showing an action replay.
Though it pains me to say it, Sky's coverage (despite Andy Gray) is superb. The BBC and Setanta are OK and ITV has always been poor.

Aside from the advert errors, their football coverage is woeful. They've got Steve Rider who couldn't muster genuine enthusiasm for football if his life depended on it. Loud, pointless pundits who achieved next to nothing in the game. Garish graphics. Highlights edited with quirky camera angles and loud music to appeal to non-fans, making it impossible to tell what's going on.

That guy wouldn't know one side of the pitch from the other as proven with his disastrous attempts at management of Spurs!
Unfortunately ITV do survive on revenue generated by advert breaks unlike Sky, Setanta and BBC so there will be times that this might happen. But in all fairness how often does it happen in relation to the amount of hours that they broadcast? 1 in 100, 1000, 10000?
