Problem With Chrome Passwords

I have been using chrome as my default browser for some time on Windows 8 and always get my passwords saved when asked. For the past few days when asked if I want chrome to save my passwords, it is saving them as username or my e-mail address, not the site I am logged into then disappear into the blue yonder.
I have checked that I am using the most recent version of chrome and clear out my cookies, browsing history etc. regularly.
Any ideas folks as I am having to write down my passwords at the moment?
You will make an almost old lady very happy!

It happened to me once a couple of weeks ago, this issue resolved by re-installing the browser.

Thanks Chris, I had wondered about this but had hoped there was a simpler solution.

Sorry for the gap in time but re-installing the browser has made no difference. I have checked that I have no rogue extensions, malaware etc. but I am no further forward!
