Provisional Drivers License.

I'm not sure where to put this but I hope this is right. Anyway, I have been seveteen for a few months now and am looking to get my provisional drivers license so I can start learning. I have a generic Driver's license form which has the option of getting a provisional on there, but you need references etc and it is very complex. I asked my friends who gave them a reference etc. and they had used a completely different form. Is there really a form specifically for provisionals or is there at least another drivers license form? mine's just a DVLA one from the Post Office. Sorry this was so long! and I'm asking out of sheer laziness!
