Getting In Touch With Old Friend

I just saw a Twitter account by someone who used to be my best friend in college. She was famously technophobic and didn't have a phone (that was six years ago), still gave me some phone number that has always been off since we parted after graduation. I'm not much better - I've still never been on social media. I gave up trying to reach her after a few weeks, haven't bothered with her for five years but recently googled her name out of interest. On Twitter she keeps saying she's very lonely and depressed. I remember what great fun we had and think - I can't just ignore the fact that she may be suffering alone somewhere. I feel like I must ask how her life has been and offer my help, I wanted to get in touch anyway. But how can I do that without sounding clingy or annoyingly obliging?
