I've always understood it to be used to refer to circumstances where something important has happened or is expected to happen - a defining moment, if you like. As far as I know it's a reference to the culmination of the bullfight in Spain: the matador uses the sword to kill the bull (a pretty defining moment) and there is literally blood inor on the sand.
