Where does the phrase 'so and so has left the building' come from? Was it first used in relation to Elvis? In what context?

It was first used by the MC at the grand ole opry when Elvis played there. After about 5 encores the only way he could get the kids to calm down and start to leave was to announce that "Ladies and Gentlemen, Elvis has now left the building"
