I believe that the phrase comes from an American quiz show like Double Your Money where the prize doubles with each correct answer. The show's highlight was the last doubling to 64,000 dollars with the attendant hype over the importance of the question. Hence the use for a really important question, probably difficult to answer.
It's an exaggeration. the original phrase, from a quiz show was 'the $64 question'. Talk about inflatiion. :-) http://www.pcgs.com/articles/article337.chtml
