why fiddlestick's end?

When somebody is thought to be wounded and the doctor comes and says "a fiddlestick's end", what does it mean? and why "fiddlestick"?

I suppose the idea is that - when you've reached the end of anything - you've reached 'no thing'. It can be used dismissively in all sorts of situations and not just medical ones.
