Im pretty sure this has nothing to do with 'old london slang',
the only feasable answer is simply the way our teeth are positioned in our mouths.
The only so called Cockney Rhyming Slang I've heard for teeth is shortened to " 'Ampsteads" from Hampstead Heath = Teeth
