Is everybody as confused as me about this one, or have I just missed the posts?

Yes, this is a tough one, devadolly and I don't think you have missed any posts. Although I have a full grid I have only figured out 8 of the 10 modifications and I'm still struggling with some of the wordplays. Nowhere near attempting the answer required to be entered beneath the puzzle yet. It's good to have a challenge though.

Very difficult. The hardest EV I've attempted. No idea what's going on.

Yes, this is very tricky. I've got a grid that's as full as I think it's meant to be, I believe I've got the nine spurious words or phrases, but if so I have a choice of ways of entering one of the medical problems -- which would then clash with a possibility for the tenth for which, in turn, I can't link to phenytoins. It's gone into the pending tray for the moment. This week's Listener was a doddle in comparison.

I have it most complete. Three lights to enter. Have 8 of the symptoms, and have sussed the title. But have drawn a blank the last couple of times.
It is really entertaining though, and agreed harder than this weeks listener.

The title is a delight. I was forced to go and explain it to the (equally bemused and sympathetic) Lady that works in the next office.

Hello Everyone,
This is definitely the hardest EV in some considerable time. Was held up in the SW corner. Have a grid which is as populated as it should be, and have linked 5 of the words that have to be removed before solving to their modified grid entries. 4 to go!

Goodness, you lot are clever, my grid is still very sparse and altho' the title is not strange, I'm no-where near finding the symptoms. A challenge, certainly Jim360.

A challenge and a half this time, but only what I expect from Ifor.The last one of his that I tackled was equally horrendous . I have managed to solve about 24 clues and have answers for 2ac and 42ac but they do noit fit in or have clashes. I am glad that others are having problems as well

At a similar stage to most of you - I have the title and the associated problem and all the other 9 (I think).

My problem is with 10 and 40 - are these thematic? My reckoning says not but what they are is a mystery to me.

X-w-f 10a is not thematic as far as I can tell and is an anagram. 40ais causing me problems as well.

Shoni70, both 2a and 42a are thematic and need "changing" before entry. In regard to 2a, 40a contains the word that needs to be removed before solving. Both are subject to the sentence in the rubric relating to the length of grid entries and space available.

x-word-fan, neither 10 nor 40 are thematic. 10 involves an anagram. I will ask you a question, how do the Travel Agents refer to passengers? The grid entry does apear in Bradfords.

Time for a preprandial Glenmorangie!

Interestingly enough the solution to EV1090 related to "All you need is Love", I thought it related to "Hey, you have to hide your love away".

Bradford has PAX under passenger, why I know not, but it sorts the clue out. I think I am now finished although the wordplay to 7 and 18 escapes me but the answers seem obvious. Didn't expect to be getting my highlighters out for this one!

Like Shoni, I'm glad I'm not the only one having problems. Beavering away, not sure if I shall finish. I find Ifor's cryptic clues more cryptic than most, anyway, without the additional hurdles to jump.
Will persevere for a while.

Aha! That time it spent festering in the pending tray did the trick, and it's now all fallen into place. A more careful reading of the preamble removed an uncertainty, and a sudden improvement in my ability to count was a bit of a help too. I agree that the title, linked with a careful reading of the preamble, is very clever.

Still swimming in mud and am afraid it will have to 'go over the shoulder' as off on hols tomorrow and only shoes in the cases as yet!!
