Fairly easy gridfill, followed by some head-scratching over the instruction. A lucky search in onelook.com's reverse dictionary led to a possibility; my reaction was at first "no way", followed by "way". Interested to see other reactions to the endgame.

My reaction to the endgame was that I couldn't believe the setter managed to get that into the grid, and so tidily as well. Blimey. Well done to Piccadilly!
I found the gridfill a bit pedestrian, but suspected that the instruction would relate to a specific area. Spotted the first few letters of what the instruction was telling me and a dive to the BRB and the endgame was resolved! Can only marvel at the finished grid.
Wow: now I need to take a breath! Great fun.
I was explaining to someone new to the mad, mad world of EVs and Listeners that I'm not usually a fan of jumbles as a device. I think it's well-justified here. Thanks Piccadilly.

I've found the disease in the obvious area - do we have to highlight it or will delineation suffice ? Interested to read how others dealt with this.

Should read "I'm fairly new to EV ...." - I was overexcited!
Wow -my admiration for Piccadilly knows no bounds. I finished the grid a couple of days ago but couldn't find the missing piece in the conundrum, so I left it for a while. Then, just now, I idly picked it up and a few letters leaped out at me. A quick scurry to the BRB with reactions almost identical to dr b's - disbelief giving way to 'yes it really is' and 'how did Piccadilly manage that?'
Very satisfying.
