This is the main point.

When they eventually arrived at the Arena, more than two hours after Salman Abedi detonated his suicide bomb on Monday 22 May 2017, the visibly frustrated fire officers were not immediately allowed on to the concourse to help because of communication errors between “risk-averse” officers in charge.//

commissioned by Andie Burnham Mayor of Manchester because of complaints that the Police had not allowed the firement to go in on account of Health and safety
In fact the fire service was out of the loop as the senior officer failed to complete an important phone call ( the one that asks, what'sup?).
The policeman broke his own rules - as they had almost declared PLATO ( keep out and dont let anyone in, someone amok with a gun ) and he said - "no it isnt PLATO you can go in"
The British Transport Police do their stuff ( they ran away in 2005) - and fifteen BR staff from the station below join the injured and deliberately disobey orders to evacuate without them.
Ambulance seemed to know what a major incident was
Dunces in the corner - Firemen - must try harder to make phone calls and not give up
and Vodaphone - must set up helplines if asked - they just didnt apparently besides have contracts to do so

Broader causes and responsibilities for the incident were outside the remit of the report, but it does mention in passing two key characters who would have featured prominently in a wider VE enquiry: Manchester's mayor and its chief of police. Survivors expressed their "gratitude" for visits to their hospital by the two dignitaries.
