How do the compilers of sudoku rate their puzzles in terms of how easy they are or how hard they are. For example some puzzles are rated from one to five. how do they rate them?

Guardian rate theirs as easy, medium and hard
I rate mine as easy, medium, hard and fiendish.
Mark. I'd love to try the fiendish, where can I get it?
The paper I used to work for published two impossible to solve ones that I know of while I was there.
At a guess I'd expect each puzzle to be based on a pre-defined pattern with a planned number of logical steps to determine a missing number, the more steps the harder.
