Leave Application Form

Hello Everyone,
I want to write an email to an employee who were on leave for 3 days. What should I write him regarding his leave application form.

Dear xyz,
You were not present on 3rd feb, 4th feb, and 5th feb. please submit your leave application form that i may update your record, and if it was a sick leave please attached your medical record with leave form as well.

'Write him' is American. British English requires 'write to him'.

I wouldn't invite someone to apply for leave retrospectively. Three days abscence without explanation, phone call, or previously authorised leave requires a one to one conversation about their behaviour. You can't just not turn up for work. What does the line manager say?
