Depending on her mood Mrs NJ replies with (something like) "I must have changed sex/I must have "identified" as male/ don't you serve ladies in this restaurant?/do I look like a "guy"?/we're leaving". (I'm sure you get the drift).
Staff in customer facing roles need to shape up and learn to address people properly. I find it absolutely infuriating that staff (not only in restaurants) have to address females as "guys". If you wonder why this is, just imagine if a mixed couple were addressed as "girls".

//Jesus. I thought we’d left Victorian etiquette back in the C20th//
More's the pity. Todays uncouth youth are quite happy to sit down to a meal wearing their baseball caps and picking up their chips and other food items with their hands whilst dipping said items in mayo or ketchup.
All dexterously achieved whilst gaping at their Iphone in the other hand. :-(

Not really encountered the being ignored thing much, couple of times maybe but then the men were ahead and thus were the ones being spoken to, I imagine if I'd have been first they'd have spoken to me. I've been included in the 'guys' thing sometimes at very informal restaurants, but at formal restaurants never.
