well, the question mentions 'hierarchy' ---- so, I am guessing here ..........

Permanent Secretary

Cabinet Secretary is number one, and in charge of the Home Civil Service, BUT possibly now this position is seen as being 'outside the hierarchy' as any recruitment is subject to the inclusion of outside applicants.
