Mail to CEO about an internal issue

I am resigning from my current job. I have kept my resignation last month. We have a quarterly incentive structure where the best performers in the team are rewarded. I was qualified for the Q4 (Oct, Nov, Dec) incentive of 2009. But the incentive amount wasnt credited to my account when rest of the members got their incentive. I had a word with my manager, which is of no use. I have decided to take this issue to the notice of my CEO. I got a very bad feeling that there is no improtance or value to employees who are resigning from their job. I just need to keep all this in a very effective manner. Can u just gimme a sample draft for the above issue which i wanna send it to my CEO.
