How does the cost of int international postage work?

You post a letter/parcel in Sydney to New York. I suppose that the Australian postage includes the cost of getting it on the plane to USA but who pays the cost of getting it delivered from the plane in US to the address.
Is there some international reciprocal agreement that each country delivers overseas post to its address free of charge or would each item incur a US charge to the AU post.
