Has anyone ever designed a questionnaire? I'm doing one and I want it to be taken seriously, so dont want it to look too amateurish. Basically the quesions have tick box answers. for example: is it possible to ......?
No never
yes under condition A
yes under condition B
yes under condition C
dont know
I want to put tick boxes down the left hand side.
I also need some boxes for sections along the lines of 'If you ticked box 2, 3 or 4 above please give details below'.
Any ideas?

Microsoft's free templates don't have a specific category for questionnaires but you might be able to modify something from the 'Quizzes & Tests' section:

Personally, I wouldn't bother with anything too complicated. It's fairly easy to produce a reasonable looking questionnaire in Word. With most fonts, you can type a 'box' (for putting a tick in) by holding down the Alt key, typing 127 on the numeric keypad, and then releasing the Alt key. (That requires the keyboard's number lock to be on, but that's the default setting anyway). It can sometimes look better if you type the 'boxes' in a slightly larger font than that used for the main text.

Ok thanks for that. I tried the ALT 127, but didn't get a tick box (I got something else), so guess that must be a shorcut to a symbol. Will have a look through them. The link to templates looks good. I reckon I'll manage to sort something with a combination of templates and bits I add in myself with text boxes, tick boxes and so on. The good thing with a template is that you're saved all the hassle of trying to get things to line up. And they do just look more professional.
