Can any ABrs tell me who decides and where to lay Fibre-Optic cable eg., Virgin or the local council?

I would imagine Virgin will decide to lay some cabing on so and so street and get permision form the council to dig the road/pavement up to lay it.
Has Virgin ever laid cable? Round Manchester, many years ago, NTL appeared to lay cable in several districts but missing many houses as they seemed to put down cable in selected areas only, and not all.
