Microsoft word 2003 why wont it let me save some words to the spell checkers Dictionary?

I often come across words when using the spell checker that the Dictionary wont allow me to "add", one being my surname and another being the word "Carer" Why is this? and can it be changed to recognise them, it allows me to save other obscure names that is not in the dictionary.

Have a look in tools > options > spelling and grammar and check your custom dictionary is set to all languages.

(I'm guessing a little, but if it's set to UK only you can't add words that are proper words in another language even if they may be valid English words too)
