If you surf the web in a Windows computer using Internet Explorer, where and how do Windows save or log your "footprints?" I know the History, Cookies, Temp, and Temporary Internet Files. Where else do Windows store your online surfing activities?

I think there is something called index.dat file too.
If you're concerned about leaving these footprints then I'd recommend using some sort of program which will erase them for you. This is guaranteed to be more thorough (and less of a chore) than trying to do so yourself manually. "Tracks Eraser Pro" is one of many such progs.
