Travel Restrictions to USA

Not having visited the States for a few years, I am wondering if anyone can help me.

Do you still get the visa form on the plane and what are the restrictions these days, i.e. carrying liquid etc.

Make sure you use the official US gov. site in applying for your visa, which will cost US$14.
