Absolutely.

I cannot conceive of the grief for this poor family -waving off their excited child on her school trip, and then seeing her again in a mortuary.

Given the propensity for 'filming every experience' which people have today, I hope that there is some footage which will help determine the cause of her entering the water - if it's a fault in the ride then it needs to be fixed before it's used again.

just going on the descriptions available in the media, probably not a fault with the equipment, per-se.

rather a fault with the operator's risk assessment; ie, was a child standing up a forseeable event, what was the likely outcome of that event, and what measures were taken to mitigate the risk?

The girl was apparently standing up at the time swapping seats with someone else. I assume the rides are not fitted with seatbelts due to the added possibility of causing injury. Accident more likely to lack of supervision/instructions to stay seated.

ermm, no there shouldn't. the hazard pertaining to a catch net is the same as requiring seat restraints on a ride like this, ie risk of entrapment in the event of overturn.

we're in danger of going off-topic here. but there's no concept of "danger" in the assessment of risk. it's all about hazard, frequency, likelihood and consequence. it might be more likely that a person may fall out IF they stand up, but the greater risk (with more serious consequences) comes from being trapped in the water under the boat.

Terribly sad. I went on one of these abroad a couple of years ago and I have to say it was bouncing about so much that I was convinced it was going to tip over. If anybody had fallen over the side you would stand no chance as all the other 'boats/rides' would just go straight over you. As soon as she stood up, which you should never ever do, and it hit the rock the poor girl would have just been catapaulted overboard. So sad.
