it's valid for you to travel, but I think kids over 5 need their own passports now
I think you need to check with the Portuguese Embassy to be sure. The only thing I can find online are a couple of travel agents sites that suggest that both you and the kids will be OK. Hardly authoritative, given that they are undated.

Ugly_bob is right in saying that children need their own passports now (of any age - not just over 5) - you cannot get an adult passport with children added to it any more. But passports already issued with children on them remain valid until the child is 16 or they expire.

So it's up to the country you are going to as to whether such a passport is acceptable. For most it probably is - the USA is the only place that I am certain doesn't allow this.
