In the section headed �Information concerning the beneficiary� �
Part 1. Fill in your Surname
Part 2. Fill in your Forenames
Part 3. Fill in your date of birth in the following format: dd/mm/yyyy
Part 4. Fill in your NHS or National Insurance number.
In the section headed �Period of validity of the form�
In Part(a) Enter your date of travel abroad, or the date you are completing the Form E111, in the following format: dd/mm/yyyy.
Note: E111 will be replaced by the European Health Insurance Card in 2005. If you tick the appropriate box on the E111 application form you do not need to apply separately for this, as the details that you have provided will be used to issue you with one automatically later in the year.
You do not need to enter anything in any other box.
I think that they may well be the identifiers for whoever approves or rejects your E111 application.
