Aims & objectives/goals chart

Not sure where to put this...
Couple of years ago my teacher had every one do a 4 square chart, a box for each of the following: aims, objectives, goals and something else... does anyone know what the last box was for and where i can find an example of this?
Its not the same as the SMART chart... that's not as good...

Any new business initiatives were subjected to an examination by a selected group of interested parties. A chart was produced documenting the discussion. It was a large square divided into four smaller squares. The smaller boxes were headed Strengths, Weaknesses, Opportunities, Threats, hence SWOT.
This is also a good one for self-assessment endorsed by a third person
