Chicken pox spots are like little blisters.

I thought chicken pox, but no spots anywhere else.. Just mouth.
Is he managing to drink or refusing? Temperature? Any inside the mouth?
How long as he had them, for the first 24 hours I only had a few spots in one place. Unfortunately got it as an adult so remember it well. Do they look like the spots on the link.
It could be many things but I'd certainly check his temperature. If it's high and it's come on this evening personally with a 3 year old I'd ring NHS direct for professional assurance (because I'd worry).
i'd go for hand foot and mouth
? Oral thrush.

They do look like the ones on the link yes.. They've appeared over the last 10-12 hours

Agree that sounds viral in origin - probably Coxsackie types - leading to either Hand, Foot & Mouth or Herpangina. Both common young children and usually resolve with little intervention other than symptomatic relief.
