Writing my dissertation on nutritional habits of jockeys and was wondering if there is a standardized questionnaire to asses knowledge of an person/athlete?

The nearest you're likely to get is the questionnaire developed by Parmenter & Wardle for the Imperial Cancer Research Fund Unit at Imperial College London. The methodology used in developing the questionnaire is detailed in great detail in the first five pages of this document, with the questionnaire itself making up the rest of it:
