In my experience I have done the following. Keep it short and to the facts. Try not to put anything personal in there one way or the other. So normally something like 'dear XXX Please accept this as notification of my termination of employment with XXXX. My last day of work will be XXXX. Yours XXX. and thats it.
Well you can take sft42's advice (I know I have been tempted a few times) but take it from me its a bad idea. I had a job I hated, but it paid really well and was local. I got another job which on the first day I know wasn't going to work out, its a long story, anyway my old boss said if I ever wanted to come back I could, so I rang him after the first day and went back a week later. So the moral of the story is if I had slagged the place off I'd now be out of a job, no money and possibly lost my house. Its still a rubbish job however.
