Aircrash Confidential - Did I Miss Something?

I watched Aircrash confidential the other night, but am puzzled. The conclusion didn't seem to fit the problem, which was a rudder servo jamming fully over with no input from the pilots. Fair enough, but the programme didn't explain how the servo moved to it's full extent in the first place. Or did I miss something?
