Apparently it is done via one's computer now and takes over 1 hour to complete, even though they know all the answers to the numerous questions they ask you.

My late mother's husband, (believe it or not) drives a mobility scooter with his guide dog walking beside him. His sight is really bad, he just sees shades.

And before anybody shouts that he shouldnt have a guide dog, I totally agree, he is a total idiot!!
Why shouldn't he have a guide dog?
AOG i think it must be different per local authority. I started the renewal process last week - i submitted my stuff online including photo on the .gov website. Then they passed it to my local council who wrote to me and asked me for certified proof of identity (and said they'd look at my CT bill for the second proof so i didnt have to send it). I paid online via my council's website

because he misuses his guide dog and puts the dog in danger of being run over and killed and the dog can not work properly walking beside a mobility scooter, they aren't trained for that!
With you on that Ratters. We sponsor a guide dog and would hate to think someone was using it with a scooter. Their training does not encompass that. I would honestly report it if I knew who it was.
