Failed Discharge

FiL had a pacemaker fitted a week ago. Discharged on Friday afternoon. 6 hours later re-admitted after 2 falls with acute kidney infection and double pneumonia.

I'm not medically qualified but imho an 82 year old man should never have been discharged - particularly as these "new" issues should not have manifested in 6 hours. 36 hours later he dies.

Any medical views, please? Particularly Sqad.

Only a suggestion, nothing more , who actually discharged him and at what time ?. So sorry .
with an actual case here, Barmaid, and ongoing, just watch out for any changes to the notes....they can pull fast ones when a cover-up is on.

Sorry to hear your news though, a shock for the family.....
not that this help you or answers your question, but it angers me when people are discharged on a friday (at 5pm!) it is the stupidest time of the week for discharging someone
Sorry for your loss, BM. I hope you get some answers soon x
I too have always understood that double pneumonia referred to infection of both lungs. I was surprised at Sqad's remark, 'whatever that is'.
Jackdaw "Double pneumonia is a laymen's term and is not a medical entity.

Pneumonia in both lungs is either lobar pneumonia or bronchopneumonia and i am surprised that the nurse used the term "double pneumon1a".........it doesn't exist in the medical parlance.
