So tonight I came into work to find a shift note from a colleague asking me to sign a finance document that I'd forgotten to sign on my last shift on the 18/8/18, her shift note was dated 21/8/18. So I proceeded to go into the locked cabinet in the presence of another colleague to sign the document only to find that the document has been signed with my signature but I never signed it! Underneath my signature was my managers signature and she dated the document 18/8/18. Now I'm a little unsure of what to do next? I hardly wanna go round laying the blame at people's door but I m so angry and upset someone has forged my signature!! Should I speak with the Police or The HR department? Surely this has to be investigated?

I am with zacs here and am failing to see what all the fuss is about. What exactly has been signed? If the document itself is accurate and you were going to sign it anyway what's the problem? What normally happens if the document isn't signed?
It depends if you think it is OK
which I have to say it doesnt seem like it does...

Is what you signed untrue ? a shift doc - so someone signed you worked 0900-1500 ? and you did ?
then leave it

OR
someone signed you worked 09 00 - 1500 and you would only have signed for 09 30- 1400 (c) ? ONLY then take action

(c) shows that a document which is false has been 'uttered' with an intent to secure payment which wasnt earned
(Fraud - s2. I think )

otherwise put it down to experience

I think yo are being anal about your signature to be honest

this is a do it yourself note on 'agency'
oh yeah but if it comes up in court
there is nothing wrong with 'these facts are true and they are signed for as true'

ah but if they arent true - then see (c)

I have read MMz post about a similar situation
it wasnt a similar situation - see option c
( sig used for things not true)
The big thing is

has anything untrue been signed for ?

if yes ( as in value attached to the statement was inflated) do something
if no ( as in the form went in on time and was correct in money terms) then no ....
