Could anyone confirm the equation to be used to calculate incidents per 1000 events if the 1000 mark was not reached?
For example; a target is set of less than 3 incidents per 1000 events. There have been 2 incidents BUT during only 297 events.

and the rule from form III is:
divide it out and multiply by a 1000
since I dont have a calculator - it is between 6 and 10 ( per thousand)
and as your teacher said back then ( 60 y for me)
do a test on something you know
2/3 - as a fraction of 100 or per cent
divide it out - 0.667 and then multipy by 100
is 67% - and we all know 2/3 as a percent is 67% near as - or 67 in 100
