Reapplying For A Driving License After A Medical Incident.

Can someone please help?? On the DVLA website it says when reapplying fill in form D1, then go to DVLA website and fill in a medical questionnaire - you have to go to a long long list of medical conditions which are in alphabetical and select yours, print and fill in. Well I don’t have one! It was declared that my seizure was a one off episode, and that I am perfectly fine to drive - I have the letter from the hospital consultant here saying that. Cani just put that with the D1 and post to the DVLA??
