So, given that your friend is clearly experiencing problems with the function of his transplanted kidney, it would indeed seem quite likely that "it's on its way out". Some people in that position are suitable for a further transplant but many are simply given the choice between haemodialysis to a schedule like the one your friend is on or daily peritoneal dialysis at home (possibly while sleeping).

thank u for info she has to go to a hospital 20 miles a away she is 50 yrs 5 stone about 5.2 height == they picked her up 4 clock in the morning got home 8 pm

She needs to ask her consultant if she's suitable for 'APD' (= automated peritoneal dialysis). If so, she can simply connect herself up to a machine each night when she goes to bed.
