What happens to the earth surrounding an underground nuclear test?

Depends on the earth, in some cases the heat and pressure etc. would cause the earth to form a ceramic in a way similar to the 'glass' beaches in the pacific.
But generally extreme heat and concussive force causing fractures and cracks but the test chambers are specially built to withstand this and are probably lined with stuff.
