I think that they are trained nurses with good experience level who go on to take a course in this specialist skill, I would imagine a lot comes from the experience of just working with people with terminal illness and having a strong vocation to help in this situation. Must be stressfull!

Have to deal with so much - pain, sickness, depression,anger, bereavement, family emotions etc.
Extremely rewarding - allowing terminally ill individuals to spend the remainder of their life in familiar surroundings and to allow them to live and to die with dignity and self worth.
