A theoretical underpinning is th basic belief that inspires people to treat mental illness in a certain way. Basically you have two major theories of old- the 'analytical school' stemming from Freud, Jung, etc believed that to make a person be less ill to reduce symptoms, you had to explore their innermost psychological recesses- thus they would prescribe analysis.

Skinner and other behaviorists believed that all behavior was a result of rewards and punishments and their interventions would be 'behavioral' in nature. So- a hyperactive child might, be assigned psychoanalysis or a behavior modification treatment plan.

Today- an offshoot of the latter is CBT Cognitive Behavioral Therapy- where though analysis is rare- there are many who still stand by the 'talking cure of the former.

the theoretical underpinning of anything is just the theory, or philosophy, on which action is based.
"Underpinnings" is an archaic word for underwear, so 'theoretical underpinnings' is just a fancy way of saying 'imaginary knickers'.
Fans self, vigorously.
what course is it that you're doing, by the way?
