do i have a disability?

hi everyone
i am filling in a form for working tax credit, and one of the questions asks do i have a disability and i don't know if i would be classed as that. i am a diabetic (insulin based), am hard of hearing and have low vision due to having laser treatment to the inside of my eyes as they are bleeding, please can someone help with the question? i have a pamphlet with the form, only it doesn't mention much about disability.

do you get disability money? if you do yes you can say on the form yes .

It's really important that you fill this form in right or you could loose out on a lot of money.Why don't you go to the nearest enquiry centre they have people to help you fill them in especially if you have visual problems.The number to ring will be in the phone book under Inland revenue, to find out where they are or the web page is www.inlandrevenue.gov.uk.As you suffer with bad health they sometimes do home visits.
